2025-2026 Permission/Registration Form

Here is the downloadable form for parents or guardians to register their children to be permitted to participate in FPCE children & youth activities for the 2025-2026 school year. This form is required to be filled out each year.

This form can be filled out using the Acrobat Reader. Then is should be saved and emailed to the address on the form. You can also print it out, fill it out by hand and deliver it to the FPCE front desk, but emailed attachments are preferred.
